Our records indicate that you have completed your last year of optometry school. We want to continue to Educate, Encourage, and Equip as you move forward in your career. We want to take this opportunity to make you aware of special membership rates. We know that as an optometry graduate you have the decision to either go into residency for one year or enter your first year of practice. So, we have two special membership options for you to choose from. If you are going into residency our resident membership rate is $100 or if you are going straight into practice our first year in practice rate is $150.
We want to update our records with any new contact information you may have. We also want to know if you have decided to go into residency. Please tell us where you are doing your residency. Complete the bottom portion of this page with your updated information, indicating your t-shirt size and the membership option for you. When we receive your response, we will send you a FREE CMDA t-shirt, along with a lapel pin that you can wear to reflect your commitment to CMDA and a membership certificate to recognize your professional association with the organization. We also want to know if our records are incorrect. If you are a student, we’ll be glad to update our records and extend your student membership if needed.
