- Return the form by email or by letter.

Transfer Stock to CMDA
Sending Stock Electronically
The easiest and most secure way to donate stock is electronically through your broker. You must initiate the transfer with your broker, who will require the following CMDA brokerage information:

Sending Stock by Postal Mail
Mail your endorsed certificate(s) and completed Stock Power Form IN SEPARATE ENVELOPES by a traceable shipping method such as FedEx, UPS, Registered Mail, etc. Deliver to:

The signed stock power form MUST HAVE A MEDALLION SIGNATURE GUARANTEE from a broker or bank.
